probably a lack of nutrients and/or poor soil structure.

if the ground hasn't been cultivated recently then you will need to 'double dig' it to break up the sub-soil. adding organic matter at the same time will help improve the structure and add some nutrients but a base dressing of balanced fertiliser is also a good idea.

oh and onions from seed take a long time to mature, use sets unless you can extend the growing season.

I grew some Sturon onion sets last season in a dustbin and had a reasonable crop not over large but it was my first time.
I intend to use well sieved soil from the pots and bags from last year and mix it will a couple of growbags and as I live right on the south coast I will plant them out in Feb/Mar and will start to pull them early Sept

spacing is also an issue, 6" between rows and 4" apart in the rows.

it's worth noting that small onions have more flavour, the 6" show winners taste crap.
